Step 5: Inserting the text fields

Create a new layer called "Text Fields". Go to the Text Options panel. If it's not visible go to Windows, Panels, and select Text Options. In the Text Options panel select Input Text from the text type pull down menu. Select Single Line from the line type pull down menu.

Select the Text tool and click to the right of the title First Name. This is where you want the users to fill in their first name.

In the Text Options enter "FirstName" for the Variable. The name of the variable should indicate clearly what the value is. Do not use any special characters here or spaces in-between words.

Do the same for Last Name and eMail. The Input Text fields are displayed as dashed lines on the canvas. If you click in a text field while using the Text tool a small box will appear in the bottom right hand side. You have the option to expand the width of any text field.

For the Feed Back title do the same as the other text fields, but select Multiline for the text type. Select Word wrap. And name the Variable "FeedBack". If you click in a text field while using the Text tool a small box will appear in the bottom right hand side. You have the option to expand the height and width of that multiline text field.
